After 30 years of practicing prosthodontics and encountering the most complex restorative, occlusal, and implant cases, I've learned that patient success hinges on mastering the fundamentals. I firmly believe that even the most advanced technology can't compensate for a lack of attention to fundamental biologic, material, and mechanical principles.
The single most critical factor in long-term restorative success is how we manage the replacement of existing dentistry. This includes meticulous caries removal, perfectly clean adhesive cores and provisionals, careful attention to the periodontal/restorative interface, and placement of definitive restorations in a pristine environment. This must be coupled with a thorough understanding of treatment planning and formulation. Combining knowledge of today’s restorative ceramic materials and margin designs with proper management of the subgingival and equagingival environment will ensure lasting success. There are specific, repeatable protocols to achieve this consistently.
